發生的原因，目前仍無定論。
不過在個人因素如吸菸、喝酒、速食、熱食、以及口腔衛生不良的人，較易發生食道癌。
環境因素如飲水及食物中含有過量的亞硝基胺，被認為是高度導致食道癌的物質；高梁、玉米、以及茶葉中的蘇酸(Tannin)，也被列為與食道癌有關的物質。
